Application form - 'Title' column on 'List of Entries' table

When a student opens an Application Form activity in their Moodle course they see a "List of Entries" table on the "Overview and Submit" page (/mod/apply/view.php) if they have previously submitted any forms.
The first column in that "List of Entries" table is called "Title" and on my Moodle site it always has the value "No Title" (see the screenshot that I've linked to below).

Where does that "Title" get set? How and where do I change it?
